Modern healthcare management takes a Care Team. The Shared Med Journal is a free app which allows you to create as many medical journals as you need to manage the healthcare of yourself and your dependents. Each journal can be shared with others, including Healthcare Providers, to form a Care Team for that particular Journal.

You can think of the journals you create as highly secured, encrypted, private groups with a timeline visible only to the Care Team members invited to it.

Each journal contains a place for storing medical history and posting updates and photos. All Care Team members invited to a particular journal are able to monitor, post and comment on entries in that specific journal.

At the user’s discretion, the app allows users to share data from a journal with the Health app and to integrate a journal with Apple's Health app for effortless tracking and to automate posting of data to the journal. Not available for iPad.

Although only one journal per device can be connected to the Apple's Health app, you can create and share a journal with a dependent who also has an iPhone. Once the Shared Med Journal is installed on their phone, that journal can then be connected to their Apple's Health app. In this way, you are able to remotely monitor the health app data of a dependent.

If the Shared Med Journal app has been prescribed by your doctor, it will include details of the doctor and their practice for easy reference.
