The Care Assistant Platform for Stroke assists with the delivery of stroke rehabilitation programs by a health service to people who have previously suffered a stroke or TIA. This program is focused on changing people's lifestyle to a more healthy and active one.

It gives clinicians the ability to access and monitor their cohort's stroke risk factors information as it is collected. This allows for review before they enter the clinic or are called for a scheduled session.
