The TeleClinical Care - Cardiac (TCC-Cardiac) app is for use by patients enrolled in the TCC-Cardiac research study.

This study has been funded by NSW Health and is sponsored by the University of NSW. The TCC-Cardiac app is designed to support patients who have recently been admitted with a heart attack or heart failure. It allows for the delivery of educational messages, the monitoring of physiological parameters (such as heart rate, blood pressure, weight and oxygen levels in the blood) by a team of hospital-based clinicians and provides a virtual cardiac rehabilitation exercise programme. The objective of the app is to educate and empower patients living with heart disease to ensure they keep well and out of hospital.
