DigiCare is an innovative digital health application funded by the German Federal Ministry of Education and Research (BMBF) and specifically developed for people with cancer. Its goal is to provide continuous, cross-sectoral support to cancer patients with high symptom burdens and sustainably improve their quality of life. DigiCare enables those affected to easily document and specifically manage their symptoms, supported by chatbots and digital questionnaires, for example, to identify their symptom progression and receive recommendations for action. The application promotes self-determination in the treatment process and provides personalized knowledge about their own illness. DigiCare connects patients, physicians, nursing staff, and especially Advanced Practice Nurses (APNs) to close gaps in care and ensure optimal care. If necessary, timely and needs-based palliative care support is integrated into the treatment process. DigiCare is aimed at cancer patients who want to better manage their symptoms in their daily lives and value connected, modern, and personal support throughout their entire treatment. The application offers digital support, modern care structures and an active role in the treatment process - for a better quality of life and safety in everyday life with their illness.
