This app is available for Aetna and Caremark members who are part of the Transform Diabetes Care program. Members in select plans may not be eligible yet, but we’re continually working to expand access.


With the CVS Health Tracker app, you can:

Pair your Bluetooth glucometer and track readings
You can pair your Bluetooth glucometer, track and sync readings, and add notes. Each time you log in, we connect to your paired meter to pull in new readings, which are displayed by date and color coded by range. The app then makes them available to the program’s care team.

The app supports the Roche Accu-Chek® Guide, Roche Accu-Chek® Guide Me, LifeScan OneTouch Verio Flex® and LifeScan OneTouch Verio Reflect®.
