⚫ How can I get death certificate in India?
Citizen can apply for the certificate at their particular Municipality/Panchayat Office by providing the Doctors Certificate and Panchanama, given by the recognized authorities like Police, Revenue Officer, etc. The service is for registration of deaths that happened less than 21 days.

⚫ Documents Required :-
✓ Age proof of the deceased – Birth certificate or SSLC certificate can submit.
✓ An affidavit specifying the date and time of death.
✓ Ration card copy.
✓ Death certificate application form.
✓ Medical Certification stating the causes of death, if required.

⚫ Birth registration is done by submitting the duly accomplished copy of the Certificate of Live Birth along with the parents' PSA marriage certificate (if the parents are married), and other documents to the office of the Local Civil Registrar (LCR) in the city or municipality where the child was born.

⚫ The information of any Death is given to the related wards on prescribed form within 21 days and then certificate is issued immediately free of cost by the person employed at Registration ward. after 30 days and till 1 year, a written permission of the Officer prescribed and on payment of late fee of Rs. 200/-.
