C4T is an application to help prepare doctors with an hospital appointment so they are well informed. In the app they find all the info they need about the patients pathway (things like timeline, learning materials, FAQs...) as well they can see their appointments and conversations. This all, so their patients hospital stay and pathway will be as smooth as possible.
