OBERBERG_FAIRsorge is a project funded by the federal innovation fund with the aim of improving cross-sector medical and nursing care for senior citizens in need of care in the Oberberg district.

Our goal is to connect medicine and care in such a way that our patients experience a better quality of life and can live at home for as long as possible. Participation does not involve any effort or costs for you.

We would like to develop innovative and secure communication channels, multidisciplinary treatment approaches and intersectoral and interdisciplinary patient care with everyone involved in the care.
