C4T is an application to help prepare patients with an hospital appointment so they are well informed. In the app they find all the info they need about their pathway (things like timeline, learning materials, FAQs...) as well they can see their appointments and conversations. This all, so their hospital stay and pathway will be as smooth as possible.
