National Health Insurance Health Passbook App Privacy Notice:

You will be linked to the "National Health Insurance Health Passbook" (hereinafter referred to as Health Passbook) application developed by the Central Health Insurance Administration of the Ministry of Health and Welfare (hereinafter referred to as the NHI) through this application. You can obtain your medical information through the Health Passbook and hand it over to us after downloading it to your mobile phone. In order to help you understand how this application collects, processes and uses your personal information, please be sure to read the following instructions patiently:

1. This application is developed by Qiaohe Digital Design Co., Ltd. and is independent of the Health Passbook developed by the NHI. There is no entrustment relationship between them. The Health Passbook is not part of this application.

2. You can freely decide whether to download your medical information from the Health Passbook application and hand it over to us. The NHI will not track how you use the information, so you must be responsible for your own decision. In order to protect your rights, we recommend that you read the usage and privacy rules of this application in detail.

3. When you decide to download your medical information and hand it over to us, the company to which this application belongs, Qiaohe Digital Design Co., Ltd., will never provide, exchange, rent or sell any personal information you hand over to other individuals, groups, private enterprises or government agencies, but this is not limited to those with legal basis.

4. To protect your information, you must enter your health passbook account number and password before downloading the information to confirm that you are the person in charge of the health passbook information. Before downloading the health passbook information, please be sure to confirm whether the content of the information is suitable for delivery to us. If you don’t want us to know, please do not download the information.

5. The health passbook information is the information provided by the medical institution to report the National Health Insurance Administration for the cost of the National Health Insurance. It is not your complete medical information, nor is it real-time information. If you think that the content of the information is different from your own understanding, it is recommended not to download it and confirm it with the medical institution where you are receiving medical treatment to avoid unnecessary trouble for you.

VI. The company to which this application belongs, Qiaohe Digital Design Co., Ltd., is implementing the "AI Comprehensive Medical Health Assessment and Medical Referral System Construction". According to the provisions of the Personal Data Protection Act, we hereby inform you of the following matters:

(II) The specific purpose of Qiaohe Digital Design Co., Ltd. collecting the data downloaded from your health passbook: to help the public and medical members to conveniently provide more complete clinical records when seeking medical treatment or referral.

(III) The category of personal data collected by Qiaohe Digital Design Co., Ltd.: C-111.

(IV) The period, region, object and method of using personal data:

1. Period: The duration of a specific purpose, the retention period stipulated in relevant laws or regulations or required for the execution of business, or the retention period stipulated for the retention of data for individual activities. (The longest period shall prevail).

2. Region: The domestic and overseas location of the "object of personal data utilization".

3. Objects: Our company (including outsourced agencies entrusted by our company to handle affairs), agencies that use data in accordance with laws and regulations, other business-related agencies, legally authorized agencies, and objects agreed by customers.

4. Methods: Use automated machines or other non-automated methods in accordance with laws and regulations related to personal data protection.
