ACO.Connect is a mobile application that allows the Michigan Pioneer ACO and its participating physicians to communicate securely, provide notification of patient status and alert physicians of changes in health care policies and regulations. ACO.Connect is a solution to enhance the Michigan Pioneer ACO’s ability to provide better health care for our aligned Medicare fee-for-service beneficiaries.


The Michigan Pioneer ACO aims to provide high quality health care services that improve the health of the population and the communities we serve. For ACO patients, our mission is to improve the health and quality of care delivered. We will continue to listen to our patients and caregivers and do whatever it takes to help patients transition through the health care system. For physicians, our mission is to provide them with the resources necessary to aid in the enhancement of patient care.

Accountable Care Organizations (ACOs) are groups of doctors, hospitals and other health care providers who come together voluntarily to give coordinated, high quality care to the Medicare patients they serve. Coordinated care helps ensure that patients – especially the chronically ill – get the right care at the right time, with the goal of avoiding unnecessary duplication of services and preventing medical errors.
