We at Clover Health have developed a mobile app for use in the hospital setting to assist in care planning for patients with respect to their hospital stay. The Clover Hospital Assistant app will assist providers in determining appropriate admission status and discharge placement if the patients should require post acute care. It does this with the assistance of
evidence-based decision algorithms to assist in determining appropriate inpatient vs observation status.
