For example, you can use it to show your family doctor (GP), pharmacist, dentist or any other healthcare professional involved in your care.

You can use it when you attend outpatient appointments, casualty or if you are admitted to hospital.

Please try to keep the information in this app up to date as it may be used to check the medications you're currently taking and what you cannot take.

However, your GP records still have the definitive list of your prescribed medication, so your GP should always be informed of any changes made.
