Sudden cardiac arrest (SCA) is the third cause of death in Europe, with an incidence of 67-170 cases per 100,000 inhabitants for events that occur outside of hospital and an increasing trend.
The timeliness of intervention and the implementation of the so-called "chain of survival" are determining factors for maximizing the chances of survival in time-dependent events.
The first link in the so-called "chain of survival" concerns the early recognition of ACR and the call to the Single European Emergency Number (NUE) 112, followed by early cardiopulmonary resuscitation (CPR) maneuvers initiated by bystanders, hopefully with the assistance of the Health Emergency Operations Center (118). Bystander CPR remains one of the key interventions to improve survival after an out-of-hospital ACR, with favorable neurological outcomes.

Effective and efficient management of the "chain of survival" is certainly helpful in promoting the survival and clinical outcome of subjects affected by ACR.
In this regard, international guidelines recommend the use of innovative technologies to help the rapid deployment of rescuers for victims of out-of-hospital ACR.
The objective of this project is, therefore, to equip the Health Emergency Operations Centers (118) with technological support which, in the event of an out-of-hospital ACR, allows on the one hand the geolocalisation of the semi-automatic defibrillators (AEDs ) closest and, on the other, the immediate and automatic alerting of the lay personnel (First Responders) present in the vicinity of the event, to verify their availability to intervene while waiting for the arrival of the emergency vehicles.
The DAE Marche app project, approved with Regional Council Resolution (DGR) n. 1711/2022 and carried out in compliance with the provisions of the Decree of the Ministry of Health of 18 March 2011 and the DGR n. 161/2017, is therefore aimed at promoting and making the use by First Responders of the AEDs present in the area more effective and efficient.
The app is fully integrated with the information systems of the Health Emergency Operations Centers (118) in the Marche region, allowing registered people to be promptly alerted if they are in the vicinity of an ACR. Whoever receives the alert can declare, via the app, their willingness to intervene and view the nearest registered AEDs and the location of the event on the map.
It is also possible, with a view to the broadest collective participation, to actively contribute to populating the AED database and reporting any problems with them.
The owner of an AED can, on his own behalf, indicate time slots and days of availability of an AED, to facilitate localization and accessibility in case of need, and manage information regarding maintenance deadlines, to allow more effective monitoring of AEDs on the territory.
Finally, the app presents a useful documentary section, where you can acquire valuable information about the airway unblocking maneuvers and cardio-pulmonary resuscitation maneuvers, the call to NUE 112, the correct use of the AED, the use of the app and other FAQs of interest.
