This application was developed especially for use on tablets, with the aim of facilitating and organizing the work of Community Health Agents (CHAs) during field activities. With a simple and intuitive interface, it brings together in one place all the tools necessary for recording actions, monitoring families and managing the micro-area, even in places without internet access.
The main features include household and individual registration, creating and organizing families, filling out forms such as home visits, procedures, collective activities and food consumption, in addition to complete management of the micro-area of activity. All of these records can be made offline, with data synchronization occurring as soon as the device connects to the internet, ensuring that all information is available for export and monitoring by the health team.
The application also offers resources that support the agent in daily organization and planning, such as productivity reports and specific modules for monitoring priority health conditions, allowing a more strategic view of the users being monitored. There is also the possibility of registering or requesting appointments directly through the app, expanding access and coordination of care.
Another important feature is the geolocation feature, which allows you to record the location of each service, enabling mapping of actions performed in the area. Based on this geographic information, the application is able to calculate optimized visit routes, organizing the agent's path according to the location of homes and contributing to more efficient travel during the workday.
Aimed at the reality of Primary Health Care, the application values the central role of the ACS, offering a practical, safe and modern solution that strengthens the bond with the community and improves the quality of health information. By combining mobility, intelligence and usability, this tool becomes a fundamental ally in organizing routines, caring for people and strengthening SUS actions.
