My medical record is your personal health record (PHR) provided by University Hospital Southampton NHS Foundation Trust. Anyone who is (or has been) a patient at University Hospital Southampton can sign-up for an account (and then login) using their NHS Login.

The app stores and displays personal and medical information, some of which will be loaded from hospital systems, including blood results, clinic letters and outpatient appointment information.

It also allows you to add your own information about your health, including details which may be being monitored as part of your current treatment, such as your weight or blood pressure.
