OPTIMUM is a diabetes management application. Using their mobile app, Diabetes Type II patients will upload their Blood Pressure, Heart Rate, Blood Glucose and Weight measurements to the Clinician Web portal. Based on these measurements, appropriate alerts are generated for the telehealth nurse. The nurse will then provide timely telephonic intervention to prevent disease deterioration, increase compliance to treatment regimen (e.g. medication), and most importantly, engage participants to better manage their own care.
