الكتاب هو دراسة لأدوار الرجال والنساء الجنسية في المجتمع، ويبحث في المفاهيم المعروفة عن الجنس وعدم المساواة الجنسية بين الرجل والمرأة في المجتمعات الإسلامية. وينطلق الكتاب من مبدأ اختلاف الفكرة والفعل أو التفكير والتطبيق في الإسلام، ويؤكد وجود فكرة المساواة الجنسية في الإسلام (بالمقارنة مع المسيحية مثلاً) مع عدم تطبيقها في المجتمعات الإسلامية، بل يتم تطبيقها بشكل عكسي في هذه المجتمعات كرد فعل لمفهوم النص الإسلامي. ويظهر ذلك في تطبيق الحدود والتحكم المفرط للمجتمعات الإسلامية وعلماء الإسلام في حرية النساء الجنسية التي يرون فيها تهديداً للمجتمع المتحضر، كما يناقش الكتاب الأسباب التي يرجع لها رد الفعل المعاكس هذا، ويناقش الأسباب وراء اعتبار حرية النساء الجنسية تهديداً، وأسباب قهر النساء الاجتماعي- الجنسي، كما يقوم برصد بعض الظواهر المؤثرة في بنية المجتمع مثل تعدد الزوجات والطلاق.
