This mobile App is a medical calculator indicating the likelihood that an elderly will have the diagnosis of frailty.

To calculate this likelihood we need to know the following patient parameters: gender, age, if they had a hospital admission in the last year, diabetes and polypharmacy (at least three different drugs).

To give this likelihood the App will calculate a total score based on the risk afforded by the above parameters. A patient will have a greater risk of frailty if that patient is female, older, has diabetes, had at least three different prescribed drugs and a hospital admission in the last year. According to the score obtained the patient will have a probability associated with frailty.

If a patient has high probability of frailty, special emphasis should be placed on them.
