HeartMan Lite is a smaller version of Personal Decision Support System for Heart Failure Management, developed from the European Union’s Horizon 2020 research and innovation programme under grant agreement no 689660.

It was then updated as a part of the "SI4CARE: Social Innovation for integrated health CARE of ageing population in ADRION Region" project, funded by INTERREG V-B Adriatic-Ionian ADRION Programme 2014-2020, 3rd Call for Proposal - Priority Axis1.
