6. If there are no questions, please select continue at the bottom of the display of requirements and descriptions
7. Select self-management or by proxy.
8. Fill in your full name, date of birth, occupation, national identification number (NIK) and current complete address, and add attachments to all the pdf files required for independent choice. and add the same entries for the data represented if they choose by proxy
9. Select collect.
