Linkage Plus works with older people to improve their wellbeing and increase their social networks and tackle isolation. It is co-funded by Tower Hamlets Council and the Tower Hamlets Clinical Commissioning Group, aiming to help people stay active and make the most of life. We offer opportunities for social, physical and learning activities and also offer outreach support to people aged 50+ in Tower Hamlets.

Why use this app: Refertohelp allows professionals in Tower Hamlets who have contact with isolated and vulnerable older people to make quick and easy referrals into the LinkAge Plus service. Refertohelp removes the need to collect and store personal information by allowing referrals to be made and sent directly through the app.

When we receive a referral we will contact the individual and arrange for an outreach worker to visit them and arrange support.
