The Ensocare Transition Mobile App allows discharge planners to securely access transition plans created in Ensocare Transition. It gives planners the flexibility to keep up with responses from post-acute care facilities when away from their workstation.

This version of the mobile app will allow hospital planners to:
• View/search their patient transition plans
• View referrals sent for each patient
• View responses received from post-acute care providers
• View the clinical documents as part of the encounter
• View the transition plan activity for each level of care
