Your Care ID will allow you to access:
NHS national systems
clinical data
patient information

This app will photograph your documents and take a photograph of your face.

You will need 2 current photo IDs from the following:
passport
driving licence
UK Biometric residence permit card
HM armed forces identity card

And you will also need 1 proof of address from the following:
Bank or credit card statement from the last 6 months
Mortgage statement from the last 12 months
Utility bill, including gas, electric, water or landline telephone from the last 6 months
Council tax statement from the last 12 months
UK driving licence (this cannot be used as both photo ID and proof of address)
