The Patient Portal App provides you with a window into your own health data by making records available on both mobile and desktop devices, and by allowing you to upload and share additional health data with your care team through an implemented advanced authorisation system. Other eHealth features can be made available directly from the patient portal, such as virtual visits, appointment scheduling, appointment reminders and direct communication with the patient's care team.
Access to a consolidated view of your health information empowers you to better manage your condition and actively participate in your healthcare. Patient Portal gives you more control with a single, quick view of all your health information. By enabling meaningful participation in decision making and sharing up-to-date medical information, patients and clinicians can work together for better outcomes. Patient Portal allows you to share health data, such as recognised steps from your mobile phone, with your personal health record.
Health features being used include Health Services and Health Management, and Activity and Fitness Tracking (e.g. StepsCadence/Steps, etc.).
